website and applications (“HealthBridge”) and associated software, and our practices for

applies to information we collect when you use any function of HealthBridge.

HealthBridge. The information we collect automatically is statistical and technical data

How We Use Your Information
The information supplied to HealthBridge by you when seeking a referral is used only to
connect you to the proper provider agency, and to provide them with the necessary
information to contact you or determine which services are most beneficial to you.
